The Ratchet Lady believes she's helping her patients by “fixing” them—but her methods can turn victims into dangerous Maniacs, creating new threats you'll have to contend with while trying to survive the woman responsible for them.

And Wolfe Asylum has treatments of its own. Pills and other medications can give you powerful benefits, but taking them comes with risks. Sometimes the cure really might be worse than the problem.

Meet The Ratchet Lady

The Ratchet Lady has a particularly disturbing idea of patient care.

Rather than simply hunting victims, she can transform them into Maniacs, adding another layer of danger as the situation inside Wolfe Asylum spirals increasingly out of control.

You'll have choices for dealing with those Maniacs—including trying to calm them down or finding more permanent solutions.
